2026 Medicare Premium Overview

Most Medicare beneficiaries pay $0 for Part A (if they worked 40+ quarters), $185/month for Part B, and an average of ~$37/month for Part D, totaling around $222/month. This does not include deductibles ($1,676 for Part A per benefit period, $257 for Part B annually) or 20% coinsurance on Part B services with no out-of-pocket cap in Original Medicare. Consider supplemental (Medigap) coverage to limit exposure.

IRMAA: Higher Income = Higher Premiums

IRMAA surcharges can more than triple your Part B premium. SSA uses your income from 2 years prior, so your 2026 premiums are based on your 2024 tax return. If your income has dropped significantly since then (retirement, job loss), you can request a reduction using SSA Form SSA-44. IRMAA is recalculated each year.

Frequently Asked Questions

Most Americans become eligible for Medicare at age 65. You're automatically enrolled if you're already receiving Social Security benefits. Otherwise, you must sign up during your Initial Enrollment Period, a 7-month window starting 3 months before your 65th birthday month. Younger people with certain disabilities or ESRD may also qualify.
